Large Swirling Sands Hourglass Potion is a tool upgrade item which can be crafted at a Timebending Table. The crafting recipe requires Hourglass level 1 to unlock.
After applying it to the Royal Hourglass it will be upgraded to remove large swirling sands in Eternity Isle.
It was added to the game in paid expansion A Rift in Time, and requires purchasing the expansion to unlock.
